Please read all of the instructions below and be sure that you understand all of the procedures for registering.  Scroll down and click on either the "Download in PDF Format" to print an application form in Adobe Acrobat Reader, or "Download in Word Format" to print an application from in Microsoft Word.  Then, mail in the application along with $100 non refundable deposit to:
